*Terms of Acceptance. The undersigned, as legal guardian of applicant understands that Hau Cam and other Wave Analytic Inc. associates shall not be liable for any injury or loss sustained by the player while playing, practicing, traveling, and participating in basketball related activities. Submitting this form shall be considered as a waiver of any claim for such injury or loss. The signee understands that you and/or your child(ren) may be photographed, videotaped, or otherwise recorded, and you agree on behalf of yourself and/or your child(ren) that Hau Cam and Wave Analytics Inc. associate may use your and/or your child(ren)’s name and likeness (in any form and without regard to distortions of character, form or color, or any other alternation) in photographs, videotapes, audiotapes, and other media, without any additional consideration to you and/or your child(ren) or to any third party. By submitting this form, I acknowledge that it constitutes a legal signature confirming that I acknowledge and agree to the above Terms of Acceptance.
